Dentistry has moved towards a minimally invasive pathway over the past years due to new technology and techniques allowing the modern dentist to produce the ideal functional and aesthetic result whilst preserving as much tooth structure as possible.
Nothing has typified this more than the recent ‘no post no crown’ philosophy and that conventional full posterior crown preparations should make way for immediate dentine sealed adhesive onlays where clinically appropriate.
Improved periodontal health and less devitalisation in comparison to complete crowns are just a few of the benefits.
When explained the advantages of such preparations patients are always impressed how dedicated we are as modern clinicians to keep as much tooth structure as possible to prolong the life span of the tooth. In my experience this has increased treatment acceptance to replace heavily restored teeth with stronger, cleaner and more aesthetic indirect ceramics.
The aim with the indirect onlay preparation is to create a strong ceramic to resin to tooth join with broad occlusal contacts that maximize the protection of the tooth being restored whilst maintaining function.
The majority of my onlay work is comprised of cuspal coverage post endodontic therapy to help protect and resist the 3D forces applied to a tooth in daily function in respect to the Von Mises Criterion as discussed by J Smithson.
